Forms And Policies

Welcome to the IAC Center. We look forward to working with you and helping you to reach your personal or family goals.

Before we start, we want to familiarize you with the Center’s procedures. Our goal is to avoid confusion or misunderstanding once we begin.

Forms – You may wish to bring these forms to your initial session to save time.

Payment and Fees

  • Counseling fees are typically set when you schedule your first session. To discuss fees contact Joni Mantell at JMantellmsw@iaccenter.com
  • When to pay:
    • Payment is typically expected at your session.
    • If you prefer to pay on a monthly basis please discuss this with your counselor. Monthly bills are due in 30 days.
    • If you require a “zero balance bill” to submit to your insurance company for reimbursement, we prefer that you make your payment at the session.
  • How to pay:
    • Payments preferred by check or cash.
    • PayPal is another option although they charge you a small additional fee. You can arrange to use PayPal by contacting Emorgan@iaccenter.com
  • Late payments: If your bill is 90 days late a $25 late fee is added to your statement and again for any subsequent month that your payment is delayed.
  • Questions about bills:
    • All billing questions must be emailed to your counselor.
    • ***Please do not leave any phone messages with billing concerns.
    • If you would like to discuss a billing issue by phone please email the question, your phone number and the best times to reach you. You can expect a prompt response.
  • FAQ’s
    • Is treatment tax-deductible? Costs for therapy and transportation to and from sessions often qualify as tax deductions. Contact your tax advisor or the IRS.
    • Is there a fee for case management services? There is a $100 an hour fee for any services delivered beyond counseling session time. This might include reviewing reports, research on your behalf, emails and telephone calls that go beyond quick questions. This does not include phone counseling.

Insurance

  • IAC Center is an out-of-network provider. While counseling is often partially covered in part by insurance companies, it is best to ask your insurer the following questions to see if you have coverage:
    • Do I have mental health benefits?
    • What is my deductible and has it been met?
    • How much does my plan cover for an out-of-network provider?
    • How many sessions per calendar year does my plan cover?
    • What is the coverage amount per therapy session? Inquire about these Procedure codes that IAC Center typically uses:
      1. Initial Consultation 90801
      2. Outpatient Psychotherapy 90834 (45-50 minutes)
      3. Outpatient Psychotherapy 90837 (60 minutes)
  • IAC Center does not accept direct insurance assignment as payment.
    • This means that you pay IAC Center in full and that you receive the reimbursement from your insurance company.
    • At the end of each month you will receive an invoice that includes all of the details that you need to submit to your insurance company.
    • If you require a “zero balance bill” to submit to your insurance company for reimbursement, we prefer that you make your payment at the session.
    • If you need any insurance forms to be completed in addition to the invoice, please let us know.

Cancelled Sessions

  • If you must cancel your session in non-emergency situations, 24-hours’ notice is required to avoid being charged.
  • A full fee is charged for missed appointments or no show cancellations with less than a 24 hour notice unless due to illness or an emergency.
  • Thank you for your consideration regarding this important matter.
